Carl Contino, MS, PT

With over two decades of experience in physical therapy practice, Carl approaches his patients with up-to-date knowledge of the field. Currently studying for his Doctorate degree in physical therapy, Carl already holds a Master’s, Bachelor’s and Associate’s degree in Physical Therapy. He received an Associate of Science in 1992 from Hahnemann University in Philadelphia, completing his internship at Northeastern Hospital the same year.

In 1998, Carl received a Bachelor of Science from Neumann University in Aston, Pennsylvania, completing additional clinical internships at Saint Agnes Hospital physical therapy inpatient/outpatient departments and Burn Center in Philadelphia. He returned to Neumann University for a Master of Science graduating in 2000. In 2002, Carl became co-owner and director of Pro-Active Physical Therapy in Philadelphia. He remained in this role until 2006, when he assumed ownership of Catalyst Physical Therapy. Catalyst Physical Therapy was originally located in Philadelphia for many years, and now operates out of Allentown, Pennsylvania. In November of 2010, Carl opened Premier Physical Therapy in Philadelphia.

Carl is a licensed physical therapist in the state of Pennsylvania, and holds a Direct Access Certificate which enables him to treat patients without a prescription for 30 days. He is certified in Functional Capacity Evaluations and Graston (Instrumented Assisted Soft Tissue Mobilization), and holds a membership in a number of professional organizations including the American Physical Therapy Association and the Pennsylvania Physical Therapy Association.

William Holl, LPT

William Holl, LPT is a licensed physical therapist in the state of Pennsylvania. He has successfully practiced in the Philadelphia area for the past 28 years. William initially graduated (Summa Cum Laude) with a Bachelor in Science, from the Catholic University of America, Washington DC in 1980. He then graduated with academic distinction from the physical therapy program at Hahnemann University in 1984.

William specializes in the management of musculoskeletal trauma resulting from work related injuries, motor vehicle accidents and other personal injuries. He is skilled in the delivery of manual therapy techniques and the set up of therapeutic exercise programs to help injured patients return to their pre injury state in a fast and efficient manner.

William is also skilled in performing and evaluating specific diagnostic testing procedures and protocols to help thoroughly evaluate and document any temporary and/or permanent impairments that may result from any sustained injuries.

Komal Patel, LPT

Komal Patel, LPT, is a licensed physical therapist in the state of Pennsylvania and Maryland. She graduated in 2008 from Sardar Patel University, Karamsad, Gujurat India. She speaks English, Gujarati and Hindi as well as some Spanish. She worked at the Amee Orthopedic Hospital in India and gained experience in orthopedic rehabilitation. There she gained experience communicating with orthopedic surgeons and nurses as well as treating various post operative and non-operative orthopedic related conditions with various levels of function.

She also volunteered at various hospitals in Maryland and she has treated a wide range of conditions such as: orthopedics, chronic pain, geriatrics, neurologic disorders, fall prevention, balance dysfunction, stroke and cumulative trauma patients.

William Metz, PT, DPT

William graduated from University of the Sciences in 2016 with his Doctorate in Physical Therapy (Cum Laude). William enjoys working with patients with various orthopedic injuries and he also likes to use manual therapy to tailor his treatment to each patient.

William was a program assistant to people with developmental disabilities, and worked with children diagnosed with ASD, Asperger’s, dyspraxia, seizure disorders, cortical blindness, and modulation/discrimination issues.

When not at work, William is an avid tennis player and instructor. He is also captain of the USciences Men’s Tennis Team and is an Ironman Triathlon athlete. Habby currently resides in Philadelphia, Pennsylvania.

Stephen Capella, DPT

Dr. Stephen J Capella, is a Doctor of Physical Therapy with over nine years of clinical experience. He graduated in 2010 from Temple University College of Health Professions. Dr. Stephen J Capella is licensed to practice by the state board in Pennsylvania and became Board certified in May of 2015 as a Geriatric Clinical Specialist; he is also Graston Technique certified. Dr. Stephen Capella also works with other doctors and physicians in medical groups including Abington-Jefferson Health System.

Dr. Stephen J Capella is currently a volunteer basketball and track coach for the Special Olympics of PA and a Shop Purple lead volunteer for the Pancreatic Cancer Action Network of Philadelphia. He is also the primary race day volunteer for the Max Fine Memorial 5K Race.

Enrique Guillen, DPT

Enrique received his Bachelor of Science in Physical Therapy in 2018 at Pontificia Universidad Catolica Madre y Maestra (PUCMM). He went on to receive his Doctorate in Physical Therapy in 2021 at College St. Scholastica in Duluth, Minnesota. He did his internship at Cibao Rehabilitation Center in the Dominican Republic. He received a Certificate of Participation for the Center of Integrated Therapy for Children with Autism Sensory Integration in November 2018. And a Certificate of Participation for the Patronage of Cibao Rehabilitation Center in the correct use of Wheelchairs in August 2018. Enrique speaks fluent Spanish and English.


Christopher Mammola, LPTA

Chris Mammola is a licensed physical therapist assistant in the state of Pennsylvania and the clinical director of Premier Physical Therapy . He is currently pursuing his Doctorate degree in Physical Therapy, and has been an employee for multiple years.

Chris graduated from Harcum College with his PTA Degree in 2007. He started as a PT technician at Pro-Active Physical Therapy in 2004, and work at several rehabilitation centers including Genesis before joining Premier Physical Therapy .

Chris is a member of the American Physical Therapy Association and has his certification in Graston (Instrument Assisted Soft Tissue Mobilization), CPR and Orthopedic rehabilitation of the knee. Chris is presently licensed for Indirect Supervision in PA and is also licensed in the state of NJ.

John Walter, LPTA

John is a licensed physical therapist assistant who joined the Premier family in February of 2015. He is a graduate of Holy Name High School in Reading, PA in 1995 and went on to Penn State University, where he received his Associate’s degree in Physical Therapy in 1997.

He has been employed full-time in outpatient therapy for the past 12 years in the Berks County area. He has also worked in the areas of acute care, sub-acute rehab, and wound care. John enjoys treating all patients but has a further interest in shoulder, knee and back therapy.

John currently resides in the Sinking Spring area with his son and dog Cooper. He is involved locally as a volunteer firefighter and EMT since 1994. In his spare time he enjoys fishing, hiking, hunting, going to the beach, and hanging out with friends.

Ashley K. Darcangelo, LPTA

Ashley is a licensed physical therapist assistant in the state of Pennsylvania. She has her degree in Physical Therapy from Penn State University, and obtained her indirect supervision license in 2017.

Prior to being a physical therapist assistant, Ashley obtained her Associate of Liberal Arts and Science from Penn State University. While in college, Ashley was on the university dean’s list, and on the Penn State Alpha Sigma Lambda Honor Society.

Ashley has a vast amount of experience in physical therapy working for numerous inpatient and outpatient rehabilitation centers. Ashley currently resides in Saint Clair, Pennsylvania.

Justin Brooks, BS, EP, CSCS

Justin Brooks, BS, EP, CSCS is an exercise physiologist who graduated from Rowan University. He is a certified strength and conditioning specialist and a personal trainer. Justin has been working in the physical therapy setting for over 10 years assisting patient care.


Mark Tyson, BS, EP

Mark Tyson, BS, EP is an exercise physiologist/personal trainer who graduated from Temple University in 1994, majoring in Exercise Physiology and minoring in Business Administration. He has been working in the physical therapy setting for over 20 years. Along with being a clinician, Mark is also our Director of Marketing and assists with the growth of our multiple locations.


Eddie Martinez, Clinical Care Coordinator/PT Aide

Eddie started working in the physical therapy field in 2008, and has been with Premier Physical Therapy since 2010. He speaks Spanish and English.

Eddie is always ready to assist patients in and out of the therapy area of a clinic. He offers aid when needed without impinging on a patient’s sense of independence and mobility. He also assists patients during their physical therapy exercises and treatments under the supervision of a physical therapist or physical therapy assistant.

Along with his good communication and organizational skills, Eddie has the ability to carry out any administrative tasks in the office.


Dennis Valencia, PTA

Graduated from Suffolk County Community College with an A.A.S in Physical Therapist Assistant in January 2021 and has been a personal trainer since 2018 which encouraged him to further his career.

Dennis’s clinical focus is on orthopedics rehabilitation, sports rehab, post op and general strengthening/condition to improve patient’s strength, mobility and address functional deficits to maximize their function.

Outside of Physical therapy I enjoy weight lifting, hiking and taking my goldendoodle on long walks.


Chris Krzaczek, PTA

Chris is one of the Physical Therapist Assistants here at Premier Physical Therapy and joined our team in December of 2022. Prior to earning his Associates Degree in Physical Therapy from Hahnemann University school of Health Sciences at Drexel University in Philadelphia, Chris was a Philadelphia Firefighter for more than 30 years.

Chris graduated from Hahnemann in May of 2000 and retired from the Philadelphia Fire Department a short time later.

Chris’s most recent job was at “JBMDL”, Joint Base McGuire AFB, Fort Dix, Lakehurst Naval Air Station and was part of the Physical Therapy Department, and found it very rewarding helping our Military Members recover from a variety of injuries and surgeries.
